“Based on staff interviews & record review, S2, and S4 disclosed out of ratio. Records show on 03/11/26 there were preschool children with staff for about minutes. S4 disclosed walking out of the classroom often to make copies or for a bathroom break leaving S2 alone with the children. This poses an immediate health and safety risk to the children in care.”
