Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on observations, interviews conducted, and record review it was determined that during opening and closing times, the children in Room and Room combine which poses a potential risk to children in care.”
“statement of S4, S8, and S15 confirming an aid was supervising few children in one room for minutes on one occasion, S6, and S7 confirmed S6 left a preschool room with 16-17 children under S7 supervision for a few minutes to assist a parent one time. S8, S11, and S15 reported hearing staff requesting assistance over walkie-talkies for being out of ratio. This is a potential risk to the health and safety of children in care.”
“interviewing staff that S1 admitted raising their voice in frustration when addressing a staff's mistake in the presence of younger children. S2, S3, and S4 witnessed or heard S1 raising their voice at staff, including one instance during parent drop-off and another while younger children were napping. S5 and S8 confirmed hearing S1 yell at a staff member in the presence of younger children and parents. This is a potential risk to the health and safety of children in care.”
