How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

CCR 101427(c) — Infant Care Food Service (c) The infant shall be fed in accordance with the individual plan.

“Based on the interview and record review, the Licensee did not meet Infant Care Food Service regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review the facility self-reported that Child # was given Child # 2’s bottle. Pertinent parties confirmed this to be true during interviews. Per Child #1’s Infant and Toddler Schedule he/she should have been given milk but he/she was given formula.”
