“Based on interviews, records obtained throughout the investigation, it was found that an co-licensee, sexually abused child(ren) in care. Co-licensee posed immediate and continuous threats to the health, safety, and personal rights to multiple children in care.”
“Per licensee's own statement the following family friend: Adult has lived at the license facility without criminal record clearance for approximately years ( 09/2021). This is an immediate Health and Safety risk to the children in care. A CIVIL PENALTY OF $500.00 DOLLARS has been assessed.”
