Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based interviews conducted and facility policies reviewed, facility did not provide a safe environment which allowed inappropriate interactions between children and staff This is an immediate health and safety risk to the children.”
“Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlet B( 9.0 ppb) and C30 (5.9 ppb). This is a potential health and safety risk to persons in care.”
“Based on director self-reporting the incident of two children left out on the playground without supervision. This is an immediate risk to the Health and Safety of children in care. Civil Penalty LIC421IM assessed. Plan of correction discussed with director”
