Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on interview and record review the children's records were missing the medical assessment forms from the child's physician and children were allowed to enroll and remain in the program beyond the thirty (30) day allotment which is a potential health, safety, or personal rights risks to the persons in care.”
“Based on interview and record review Child (C1) tried to help get a toy from underneath the changing table when their left hand was caught underneath the table. C1 sustained an injury to the left finger fingernail. Staff provided aid and reported the incident to the parent, but did not report the incident to the Community Care Licensing (CCL) office which is a potential health, safety, or personal rights risk to the persons in care.”
“Based on observation interview and record reivew. LPA observed the diaper changing table inside the classroom with two gapped openings at the bottom on each side. The openings are in reach of the children and they can place their hand inside causing entrapment and/or injury which is a potential health, safety, or personal rights risk to the persons in care”
