“... Based upon interviews with children, staff members and parents of enrolled children, it was found that out of children stated they had witnessed a teacher grabbing a child by the arm in a forceful manner which is a potential health, safety and personal rights risk to children in care.”

Measure have been taken by the Licensee and Director to exterminate them but evidence shows the problem has persisted over a period of time. This is a potential risk to the health, personal rights, and safety of children in care.”

“Based on interview, observation, and review of relevant documents, staff and Director had knowledge of the insect infestation since at least 7/18/22 and did not take all measures to rid them inside the facility until 8/9/22. This is a potential risk to the health and safety of children in care. Since the onset of the investigation, the Director has scheduled and provided proof of pest control services provided on 8/10/22 and 8/27/22 for the playground and inside the facility to remove insects on the premises. Plan of correction has been completed by 8/27/22.”
