Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“LPAs observing crib and play yards with no fitted crib sheets. LPAs saw Licensee place an infant in a crib with no sheet and on a dirty mattress. This poses an immediate health, safety, or personal rights risks to children in care.”
“Licensee did not ensure that staff had a fingerprint clearance prior to working in the home. Staff provided care and supervision for a child in the facility for one working day. This is an immediate risk to the health and safety of children in care.”
“Licensee’s home had excessive clutter and debris which has the potential to cause safety hazards. There were areas of the home with unsanitary conditions due to dirt, clutter and dog feces. Licensee’s dog bit the pants of a law enforcement officer on 10/8/2025. This is an immediate risk to the health and safety of children in care.”
