“interviews which revealed child #1 left the premises of the licensed family child care home without the licensee’s knowledge. Child #1 wandered from the licensed day care home located at 1323 S. Thornburg Street crossed the streets of Thornburg street and Ronald Place. Child #1 missing from the licensed day care for approximately minutes. This Poses an immediate Health, Safety, or Personal Rights risk to children in care. An immediate civil penalty assessed $500.00 for failure to supervise children appropriately.”
