“Based on infromation obtained Director sent out a notice of increase child monthly rates to parents on February 8, 2024 effective March 4, 2024. Director failed to give a required day written notice. This poses a potential risk to the health and safety of children in care.”

S1 who was providing care to children enrolled did not obtain criminal record and child abuse index clearances prior to working in the facility.
