Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on LPA observation, there were children in the Preschool A room supervised by only one teacher and children in the TK room supervised by only one teacher. It was nap time but not all children were napping which poses a potential health, safety, or personal rights risk to children in care.”
“Based on interviews conducted with AD, S4, S6, S7 & P1, adequate supervision was not provided to children in care, resulting in a C2 eating a crayon which poses a potential health, safety and/or personal rights risk(s) to children in care.”
“Based on interviews with staff and parents, child was suspected of having Hand, Foot, and Mouth Disease on 7/16/25 and was moved to a classroom belonging to the infant program instead of the center's isolation area which poses a potential health, safety, or personal rights risk to persons in care.”
