How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

CCR 102402(a)(3) — The Department shall have the authority to suspend or revoke any license for the following reasons:

“Based on LS statement on 02/06/26 she left home at 6:45 am, however, LPA observed LS arrive at 8:23 am. A1, A3-A5, A7-A10 statements don't corroborate with LS statement of facility closure from 11/5/25 to 11/11/25. A2’s statement that play yards are not used doesn't corroborate with LPA’s observation of children napping in play yards which poses/posed an immediate health, safety, and/or personal rights risk to children in care.”
