A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The Office of Child Care Licensing received a complaint on August 4, 2026, regarding allegations of lack of supervision that resulted in two children leaving the center without staff knowledge approximately six months to a year ago and walking to their home in a nearby apartment complex. There was no written documentation of this incident at the facility. An investigation began on August 10, 2026, and concluded on August 21, 2026, including interviewing a staff member and the administrator. The staff member interviewed acknowledged propping the door open, and two children leaving the center without being noticed by the remaining staff member. An additional concern noted during this investigation was the failure to notify OCCL when a child(ren) were missing while in the center s care. Findings: Based on the results of the investigation, this complaint was: Substantiated.”
“OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received on June 10, 2026. On June 5, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of not using positive behavior management, resulting in bruising left on a child. An investigation was conducted beginning June 11, 2026, and concluding on June 29, 2026. This complaint is substantiated for the use of prohibited acts of behavior management by both staff present at the facility on June 3, 2026, based on review of video footage and interviews conducted. Video footage for approximately one hour and forty-five minutes was reviewed, from 9:19 am 10:30 am, and from 1:57 pm to 2:30 pm. During this time span, Staff #1 is observed pulling multiple children by one arm at least 33 times. Additionally, Staff #1 pushes children multiple times, causing at least one child to hit their head on a bookshelf. Staff #1 uses restraint on two children, one of which Staff #1 holds onto their lap, slaps in the face, and yanks their head down by their hair. Staff #1 threatens to spray a child in the face with an unknown substance. Staff #1 is heard speaking in a threatening and demeaning tone several times, including telling a child I will fuck you up, and I will whoop your fucking ass, and takes a child into the bathroom alone for a period of time. One child is observed to be in time-out, consisting of a combination of being restrained by Staff #1 and being made to stand facing a wall for a total of 45 minutes. Another child was observed to be in time-out for a period of at least 11 minutes. Additionally, Staff #1 pointed their finger in a child s face multiple times, held a child over their head while speaking inappropriately, made negative comments about children, and jabbed a child in the sternum while yelling. Staff #2 was seen dumping a child onto the classroom floor from their nap cot and placing their leg over the child during nap time . Staff #2 was present in the classroom during some of these noted actions of Staff #1. When interviewed, Staff #2 denied seeing anything inappropriate occur in the classroom on June 3, 2026. Multiple additional concerns were noted during this investigation. This includes the failure of staff to follow the requirements for reporting suspected abuse and neglect from a staff member. Staff #2 was present in the classroom during inappropriate actions from Staff #1 and failed to say anything or make a report to the Abuse and Neglect Report Line. When interviewed, Staff #2, Staff #4, and Administrator #1 did not note the proper procedure for handling suspected abuse and neglect, all noting they would contact Administrator #1 rather than call the Abuse and Neglect Report Line. Staff #2 denied recognizing any inappropriate behavior from Staff #1 in the classroom. An additional concern is not having an understanding of and respect for children and their needs, including a respect for a child s family and culture. During nap time, Staff #1 and Staff #2 engage in conversation in front of children who are awake, calling children names and demeaning their families, including comparisons of racial background, noting the children are not disciplined. The overall actions and conversation in the video footage show a lack of understanding or respect for the needs of the children in care. In the interview with Administrator #1, they commented on the children s negative actions when recounting their review of video footage, noting their observation was the children did not want to sit on the carpet or did not want to lay and be quiet during nap time. An additional concern is the chronic use of personal cellphones while in the classroom. Staff #1 repeatedly used their cellphone while responsible for caring for children, including a video chat with someone while alone with the children. Staff #2 is observed using their cellphone during nap time, and when interviewed, stated they only use it to contact families. An additional concern is lack of supervision, as both staff members are observed leaving the classroom unsupervised during the footage. When interviewed, Staff #2 stated that due to limited staff, classrooms are sometimes out of staff-to-child ratio. Additionally, at one point a child climbs onto a bookshelf and walks on it, and Staff #1 acknowledges the action but is heard stating, it s fine, [they] can hurt [themselves]. An additional concern noted was a lack of materials available in the classroom based on video footage and direct observation of the classroom. There were two puzzles, one bin of blocks, some dramatic play materials and dolls, one or two music toys, and some color sorting bins. Additional concerns include the lack of an activity schedule, not following a lesson plan, failure to provide appropriate interactions with preschool-age children, and not following sleep requirements. The children were required to remain on their nap cots for a period of four hours on June 3, 2026. Video footage from the morning shows a lack of positive interactions with the children, and lack of guidance, structure, or clear expectations. Children were told to sit on the carpet after completing breakfast and were disciplined for not remaining still on the carpet for an extended period of time with no guidance or activity. An additional concern is the administrator not being present the required number of hours based on interviews and lack of documentation. Staff interviewed each answered differently regarding the amount of time Administrator #1 is present, with one staff member stating they are present only to an hour daily before going to the other associated facility. No attendance documentation for Administrator #1 was available. An additional concern is the lack of a working telephone for the facility. An additional concern noted was receipt of information from Adult #1 regarding an injury to Child #1 occurring in July 2025, in”
“The Office of Child Care Licensing (OCCL) received a complaint regarding allegations of the center not notifying OCCL of a loss of electricity, not maintaining the appropriate classroom temperature, and not being able to prepare meals for the children. Unannounced visits were conducted on February 6 and 9, 2026. The complaint is substantiated for not notifying OCCL of a loss of electricity. Owner #1 and four staff members stated there was a full loss of electricity for approximately one hour on February 5, 2026, and a partial power outage from February 5 through 6, 2026. There is no evidence to substantiate this complaint for not maintaining appropriate classroom temperature based on the interviews conducted. Owner #1 and three staff members stated there was no impact to the temperature of the classrooms. There is no evidence to substantiate the allegation that meals had not been served because Staff #2 stated all meals had been served on February 5, 2026. An additional concern noted during this investigation is the center did not have a person trained in day-to-day operations available during center hours when the administrator is absent from the center.”
