“Ensure the gates in the outdoor play area are self-closing and self-latching. The two exterior gates in the outdoor play area were observed not self-closing. Repair the gates to ensure they self-close and self-latch.”

“Ensure all areas are clean. Clean the vent located on the lower wall just outside the bathroom in Classroom 1.”

“Ensure staff attendance includes exact hours worked in each classroom. Staff #1 was present in classroom 2; but was not signed into the classroom. This was discussed and corrected when the staff member signed into the classroom.”

Four staff members need annual review of abuse and neglect, an application or resume, job description, two references, employee declaration, drug/alcohol statement, receipt from fingerprinting, eligibility letter from CHU, adult abuse registry check, adult health appraisal with TB results, and release of employment history. One staff member needs an eligibility letter from CHU. One staff member needs service letters.”
