A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Ensure staff files are complete. One staff member needs CPR and first aid certification. Twelve staff members need first aid certification. Two staff members need administration of medication training/certification. Thirteen staff members need receipt of verifications from fingerprinting and eligibility letters. One staff member needs a health appraisal.”
