Records held from Sep 27, 2021 onward. Earlier visits may exist in the agency’s own files.

Delaware publishes only visits where a regulation was cited, plus complaint investigations. Visits that found no violations do not appear in the state's data, so a gap between dates here does not mean the program went uninspected. Delaware does not publish a severity weight for its citations.

How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

Substantiated complaint

“On April 19, 2022, a complaint was received regarding serving spoiled milk, not meeting the needs of a child in care, and a parent not being informed of a child s injury or receiving an incident report. An investigation was conducted on April 26, 2022. There was no evidence to substantiate this complaint for serving a child spoiled milk because the provider stated she keeps sippy cups at the home, washes them out, and she had no knowledge of a sippy cup or bottle in a child s bag. There is no evidence to substantiate the claim that [name withheld] failed to meet the needs of a child because he missed two Zoom sessions because it was due to lack of communication and technical difficulties. This complaint is substantiated for failing to inform a parent or guardian of an injury while in care and providing documentation of the incident.”
