Inspection history

The city notes that some violations may not be final: programs can challenge citations at a hearing, and adjudicated violations may be dismissed.

Violations
Text reproduced verbatim · NYC Department of Health and Mental HygieneNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“At time of inspection it was determined that child care service failed to take appropriate action to keep facility free of pests or conditions conducive to pests. Child care service fails to maintain extermination service log.”
“At time of inspection floors/walls ceilings were observed Not maintained; in disrepair or covered in a toxic finish.”
“Equipment and fixtures maintained clean and in good repair”
“At time of inspection toxic substances observed improperly stored so as to result in the potential for exposure or contamination of facility surfaces.”
