How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

5180:2-13-12 — Safe Environment

“Children in care shall be protected from any items and conditions which threaten their health, safety, and well-being. During the inspection, it was determined the program did not protect children from an unsafe item, condition or equipment. The following was observed: Upon entering the home, it was observed that renovation and flooring repair was underway. The flooring project appeared to be incomplete, open floor vents were observed without protective vent covers, creating potential safety hazards. Various tools, equipment, and materials associated with the renovation were present on the floor and throughout the home. Theses items included a dolly, a shovel and other construction- related tools and machinery. Additionally, large containers of chemicals or potentially hazardous substance were observed in the residence. In another room, children's daycare toys, play materials, and other child care supplies were stacked on top of one another. The quantity and arrangement of these items significantly obstructed access to the room and prevented normal use of the space. Also it was blocking one of the emergency exits in the home. Any hazardous equipment must be removed, replaced, or repaired and any hazardous condition must be corrected and must be made inaccessible to children. Submit the program’s corrective action plan, which includes a statement that the item or condition has been removed, to verify compliance with the requirements of this rule.”
