Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“During the inspection, and a review of children(s) files, it was determined that the health record of each child did not include immunization records.”
“During the inspection, and a review of children's files, it was determined that the medical statements were not updated every thirteen months from the date of examination”
“During the inspection, and a review of children's files, it was determined that a medical was not provided for a child three years old or older within 30 days of admission and/or the examination did not occur within twelve months prior to the date of admission.”
