Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
Ohio Department of Children and Youth publishes these findings only as a scanned report. The text below was transcribed from that scan — the archived original is authoritative.
“In review of the children’s records, it was determined the program did not meet the requirements for caring for at least one child, indicated on the Children's Records Review, with a condition that requires a DCY 01236 "Health Care Plan Documentation & Permission to Administer Medication" or equivalent form. The following was observed: *There was no plan on file. *Signature of parent granting permission to administer medication and was missing. Provide staff training. Submit the program’s corrective action plan, which includes a copy of the completed DCY 01236 or equivalent form, to the Department to verify compliance with the requirements of this rule.”
“During the inspection, it was determined that information was not up to date in the Ohio statewide licensing system. The following information was not current: The private pay rates had not been updated. Submit the program's corrective action plan to the Department to verify compliance with the requirement of the rule.”
“During the inspection, it was determined that indoor swings, slides, climbers, and climbing apparatus did not have required shock absorbent protective covering under and around the equipment. Secure current training for the area needed or restructure the staff who are currently trained on the vehicle during routine trips and field trips. Submit the program’s corrective action plan to the Department to verify compliance with the requirements of this rule.”
“During the inspection, it was determined that open containers of ready to serve food and concentrated formula were not stored appropriately.
