Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
Ohio Department of Children and Youth publishes these findings only as a scanned report. The text below was transcribed from that scan — the archived original is authoritative.
“During the inspection, a ratio of 1 child care staff member(s) for 24 children was determined to have occurred for the school age group. The following was observed: A child care staff member stepped out of the room. A staff member was in the kitchen, leaving the other staff member alone with all of the children. Additional child care staff members must be hired or current child care staff members must be rescheduled to maintain compliance. Provide staff training. Submit the program’s corrective action plan, which includes a statement that training was provided, to the Department to verify compliance with the requirements of this rule.”
“During the inspection, it was determined the program’s emergency preparedness and response plan (EPRP) did not meet the requirement or was missing the information. The following was observed as missing: Medical or dental emergencies, including emergency transportation. The annual training was complete and documented as required. Make the necessary revisions to the EPRP. Submit the program’s corrective action plan, which includes the revised information, to the Department to verify compliance with the requirements of this rule.”
