Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
Ohio Department of Children and Youth publishes these findings only as a scanned report. The text below was transcribed from that scan — the archived original is authoritative.
“During the inspection, it was determined that the program was using space for child care ina manner that had not been approved by the Ohio Department of Commerce or the local certified building authority. The following condition(s) were observed: The Preschool room occupancy had been exceeded. 9 children were being cared for in this space that had been approved for 7 children. The School age room occupancy had been exceeded. 14 children were being cared for in this space that had been approved for 10 children. Submit the program’s corrective action plan, which includes building approval for use of this space, a written statement that the building occupancy limitations are being maintained, or a written statement that it is no longer being used, to the Department to verify compliance with the requirements of this rule.”
“During the inspection, a ratio of 1 child care staff member(s) for 9 children was determined to have occurred for the Toddler/Preschool group. The following was observed: Two groups were combined, and the program did not follow the ratio for the youngest child in the group. Additional child care staff members must be hired or current child care staff members must be rescheduled to maintain compliance. Provide staff training. Submit the program’s corrective action plan, which includes a statement that training was provided, to the Department to verify compliance with the requirements of this rule.”
“During the inspection, it was determined that handwashing requirements were not followed as required in rule. The following was observed: At least one staff/child did not wash their hands after each diaper change or pull-up change. Submit the program’s corrective action plan to the Department to verify compliance with the requirements of this rule.”
