Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
Ohio Department of Children and Youth publishes these findings only as a scanned report. The text below was transcribed from that scan — the archived original is authoritative.
“During the inspection it was determined that Appendix A of this rule, “Staff/Child Ratios, Age Grouping and Maximum Group Size”, was not posted in a noticeable area in the program. Submit the program’s corrective action plan to the Department to verify compliance with the requirements of this rule.”

The following was observed: A staff member had a background check on file from another program. Submit the program's corrective action plan, which includes a statement that the correct form is now on file, to the Department to verify compliance with the requirements of this rule.”
“In review of the staff records, it was determined that the immunization records for 3 of 6 employees, including child care staff members, did not meet the requirement. The following was observed: Not all staff have immunization records on file. Submit the program's corrective action plan to the Department to verify compliance with the requirements of this rule.”
“In review of the staff records, it was determined that the tuberculosis (TB) screening for 1 of 6 employees, including child care staff members, did not meet the requirement. The following was observed: Not all staff members have a TB screening on file. Submit the program's corrective action plan to the Department to verify compliance with the requirements of this rule.”
“During the inspection, it was determined the program’s written policies and procedures provided to the parents/guardians and employees was missing an item(s). The following was observed as missing: -Scheduled closings and basic daily schedule. -Opportunities for parents to meet with teachers regarding their child. -Releasing child to people other than the parent, releasing a child according to a custody agreement, follow up when a child scheduled to arrive from another program or activity does not arrive. -Summary of procedures taken in the event of an emergency, serious illness or injury. -Transportation policy for routine walking trips and emergencies, including if the center will provide child care services to children whose parents refuse to grant consent for transportation to the source of emergency treatment. -Policy to prohibit center employees from exposing children to inappropriate language, media, or behavior. -Ensure compliance with the Americans with Disabilities Act (ADA), including administering medication and/or care procedures to children with disabilities. -Formal screenings and assessments on enrolled children and if the program reports child level data to DCY pursuant to Chapter 5180:2-17 of the Administrative Code. -Social media policy including, but not limited to, addressing publication of child photos or other identifying information, if applicable. -Usage of emergency medical devices and emergency medications, including but was not limited to Automated External Defibrillators (AED) and Naloxone, if applicable. -Legally required information for parents and employees found in appendix C to this rule must be included with the parent/employee handbook. Revise the program’s written policies and procedures to include the missing information. Submit the program’s corrective action plan, which includes the revised written policies and procedures, to the Department to verify compliance with the requirements of this rule.”
