Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The backyard play area was observed to contain broken toys, which can pose a potential risk to children in care.”
“One cat's vaccination was due by 12/14/24.”
“A room where children were in care lacked a smoke detector. This was corrected at the inspection when the provider's spouse installed it.”
“A child's record lacking the admission form also lacked the notification of liability insurance form. This was corrected at the inspection when the parent sent the signed form via text message to the provider.”
“One cat lacked the updated vaccinations. It was due 12/30/23.”
“The admission forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023. There was no admission record on file for a child in care.”
“One child's record did not have a complete immunization record. It only had the most recent immunizations received.”
“Three children required the health statement from the health care professional. There was no documentation of it in the files.”
“The operational policies did not have a written policy for promotion of physical activity.”
“There were 3 children, who met the criteria for 747.633, did not have a vision and hearing screening on file.”
“One child's record did not have an immunization record on file.”
