Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the review of the operational policies, the policies were incomplete for parents rights, promotion of outdoor activities, suspension, expulsion.”
“The enrollment agreement reviewed was not the updated form. The record of a child was missing the water safety permission, the medical authorization and the emergency contact information.”
“The provider did not have a written activity plan available for review.”
“While conducting an assessment it was found the operation did not update liability insurance by the annual anniversary date of 10/08/21.”
“A child's file was found to not have immunization records.”
“Cleaning items were observed on a shelf in a bathroom used by children in care, these were accessible to the children. This was corrected at inspection when the cleaning items were removed and stored out of reach of the children in care.”
“The bathroom used by the children in care was found to have a trash can with no lid and trash accessible to the children. This was corrected at inspection when the trash can was removed from the bathroom.”
“The operation was found to not have completed the required form indicating the Consumer Product Safety site had been reviewed. This was corrected at inspection when the operation completed and posted the required form.”
