Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the investigation, it was determined that a caregiver repeatedly pushed down on a child's back and head in an attempt to make the child lie down on their cot during rest time.”
“Menu substitutions were being made without being recorded. This was corrected at inspection when a discussion was had with the person in charge about tracking menu substitutions and the substitutsions for today were logged.”
“Four infants who are not on table food did not have current feeding instructions.”
“A caregiver observed in the classroom did not have current training in SIDS/Shaken Baby Syndrome/Early Childhood Brain Development. This was corrected when the caregiver completed the training while I was present at the operation.”
“Infant feeding instructions are not being updated monthly.”
“Two of ten children's files did not have current health statements.”
“One of ten staff's first aid training was expired.”
“One of ten children's files did not have an updated immunization record.”
“Two caregivers did not have CPR/First-Aid training.”
“One child did not have their hands washed after their diaper was changed. Note: This was CORRECTED AT INSPECTION when the caregiver assisted the child to wash their hands.”
“8 out of 10 children's records reviewed were missing completed special needs statements.”
