Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of ten children's files evaluated is missing the address of the emergency contact. Note: Corrected when the parent completed the address of the emergency contact.”
“In the infants classroom, I observed three infants, ages three to four months, sleeping in swings.”
“One out of two transportation vehicles used to transport children was missing a first aid kit. Note: Corrected when the director added a first aid kit to the vehicle.”
“In the Wobblers, Two's, and Pre-todd's classroom, electric fans were on the floor and were accessible to children. Note: Corrected when the fans were removed from the classrooms.”
“Two out of ten children's files evaluated did not have current immunization records on file for review.”
“The hooks for backpack storage outside of the Pre-Todd's and Three's classrooms were not labeled with the children's names. Note: Corrected when the hooks were labeled.”
“In the Pre-todd classroom, a basket with multiple bottles of sunscreen were in an unlocked drawer that was accessible to children. Note: Corrected when the products were moved out of the children's reach.”
“In the Mini Wobblers classroom, I observed a caregiver wipe a child's nose then proceed to conduct other duties without washing hands.”
“Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support violations of this standard.”
“One transportation vehicle's safety alarm was not working according the manufacturer's instructions as it did not sound while being evaluated.”
“On the main playground, I observed a screw protruding from the children's play structure. Note: Corrected when the screw was fixed to the manufacturers instructions.”
“The operation has documentation showing the last lock down drill was conducted on 1/12/2022.”
“In the mini wobblers classroom, I observed multiple bottles that were not labeled with the children's names. Note: Corrected when the caregiver labeled the bottles.”
“Based on information obtained, there was sufficient evidence to support food was served in an unsanitary manner in the infant classroom.”
“Based on information received during the investigation interviews, there is sufficient information to conclude that the caregiver did not properly supervise an infant in care.”
“4 out of 8 caregivers are missing 10 of the required 24 hours of annual training.”
“Two children were observed asleep in their rockers. Note: This was corrected when the children were removed and placed in their cribs.”
