Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was left unsupervised when they went into the restroom after lining up to go to another classroom.”
“Two EPI pen users did not have the FARE form filled out by the parent and physician.”
“One staff member was present without a cleared background check.”
“2 rabbits were present without a health statement from the vet.”
“Caregiver was observed in classroom with direct access to children without an active background check. Caregiver was sent home.”
“It was reported that caregiver pulled child by the arm causing injury.”
“The operation did not report an injury requiring medical attention to licensing.”
“A 7 month old was observed sleeping in a restrictive device. Infant was removed and placed in his crib.”
“Based on a DFPS investigation, it was determined a caregiver did not supervise in an appropriate manner. There was a child biting incident earlier in the day. The caregiver did not make adjustments with supervision while talking with a parent, and the same children were involved in a second biting incident resulting in a child received multiple bites at different places on the body. The caregiver was not in a position to be able to intervene once the biting started.”
“DFPS determined in the investigation, that there were 15 children with one caregiver with children ranging 18-24 months. The most children one caregiver can have with this age group is 9.”
“Based on a DFPS investigation, this standard was reviewed and found deficient. A caregiver did not demonstrate competency when the caregiver did not follow operational policies to account for children during a transition time.”
“Based on a DFPS investigation, this standard was reviewed and found deficient. A caregiver left a child alone outside for 9-10 minutes.”
“This standard was evaluated in a DFPS investigation and found deficient. It was determined a child was neglected while in care.”
“An infant was observed sleeping in a swing while in care.”
“This was reviewed in a DFPS investigation and found to be deficient. A caregiver was reported to administration for using a vape cigarette while on the property.”
“This standard was reviewed in part of a DFPS investigation, and found to be deficient. A caregiver placed a child in a high chair for longer than appropriate time frame.”
“This standard was reviewed in part of a DFPS investigation and found to be deficient. A caregiver did not use good judgment when they placed a child in a separate area away from direct supervision as a form of punishment.”
“The operation received a finding of Reason to believe regarding a child being neglectfully supervised while in care.”
“Three children were found outside of the assigned play area, and one of the children was found outside of the fenced area in a bus on the operation property.”
“The assigned group had 2 caregivers, the group split, and each caregiver did not know which children they were responsible for. The children were sent from the porch to the outdoor play area without any knowledge of children assigned to the particular caregiver.”
“The director did not ensure corrections were made to gates that were not closing properly and/or had gaps that a child could fit thru.”
“Two infants under the age of twelve months were observed sleeping with blankets around them in the crib.”
“A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.”
“Caregivers CPR training was completed online only without hands on instruction or testing.”
“The operation had a director change and it was not reported within five days.”
“This standard was reviewed in part of a DFPS investigation and found to be deficient. 1) The caregivers used a swing to feed an infant in care who was not supposed to be in a swing. 2) The director at the time of the incident, provided false information to parents regarding the incident.”
“This standard was reviewed in part of a DFPS investigation and found to be deficient. A caregiver shook an infant in care.”
