“Based on information gathered, it was determined that caregiver gave a child water in a sippy cup that was not thoroughly washed from the inside of the valve/collar that had mold in the valve/collar of the sippy cup, therefore water was not served in a safe and sanitary manner. Note: This was corrected prior to inspection when the operation only uses disposable cups, and all other extra sippy cup were disposed.”

“The caregivers in three classrooms reported the incorrect number of children in their classrooms.”

“At time of inspection, a caregiver in a Preschool class was working directly with children without having received a determination of eligibility to be present at the operation. This was corrected at inspection as caregiver was removed from class.”

“It is found during an investigation that a caregiver failed to positively redirect a child's behavior by forcibly picking the child up by the arms and onto their feet to move them from the area.”

“The operation did not complete the requirement of reporting the number of employees that left in 2024 by 1/15/2025.”

“The caregiver did not demonstrate good judgement when an infant was placed in front of a sprinkler that had hot standing water in it. The hot water was not drained and sprayed the child causing burns.”
