A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“A child under the age of 24 months was in the same room with the television that was showing a program on the screen.”
“Children's files have not been updated.”
“The proof of request for background check that is required to have in your personnel file is missing.”
“Operation did not have fire drill documented for every month. Operation only had four fire drills documented. Permit holder stated they had completed four fire drills. Also, the documentation shows they are not exiting in less than three minutes.”
