Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children were found unsupervised in a classroom after separating from their group.”
“A child went to the restroom, but when the child returned, the caregiver and students were gone, leaving the child unsupervised.”
“It was determined that a child enrolled in the operation did not receive a scheduled lunch meal during a break camp day, despite being signed up for the lunch package.”
“Based on the evidence gathered through the investigation, two children were left unsupervised on the playground for 8 minutes.”
“Based on the evidence gathered during the investigation, a child was left unsupervised for about 10 minutes.”
“An incident/illness report form with the required information was not completed when the child was injured in care. This was corrected when the operation completed the incident report.”
“A self-report was not made in a timely manner. This was corrected when the operation contacted CCR to make the self-report.”
“An incident/ illness report was not shared with parents within 48 hours of when the incident occurred. This was corrected when the operation emailed the parents a copy of an incident report and statement four days after the incident.”
“Out of ten staff files reviewed nine were missing proff of education.”
