Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child out of four did not have immunizations on file.”
“Three of the four children records were missing a selection on health statement.”
“The director stated she completed First Aid/CPR all on line without a manikin.”
“The operation had not done a fire drill in FEB and MAR of 2025. This was corrected at inspection when the operation conducted a fire drill.”
“The operation did not have a policy on promotional of indoor and outdoor physical activity.”
“The operation did not have a policy for infant safe sleep in operational policy.”
“The playground space being used for the daycare does not have two exits.”
“The operational policies did not have the promotion of indoor and outdoor activity play requirements.”
“Two out of five children's records reviewed were lacking the parent signature for the emergency medical transportation section.”
“Three out of five children's files were lacking the address for the emergency contact and two files were lacking an emergency contact.”
