Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the evidence obtained, a child was playing on monkey bars during a field trip which resulted into an injury.”
“Based on the evidence obtained, the operation did not self-report to CCR within the required timeframe.”
“Based on the information obtained, there is sufficient evidence of a caregiver not demonstrating good judgement.”
“The school-aged play area has broken wood pieces that are splintering on the climbing wall and other surfacing areas that is a risk to children and can cause splinter and tearing of clothing.”
“In the three year old classroom the outlet covers were missing and accessible to children. This was corrected at inspection once the outlet covers were placed.”
“During a walkthrough there was a person present in the operation without an active background check. This was corrected at the inspection when the background check process was started and the person was sent home.”
“While doing a walkthrough at the operation a caregiver was observed not washing hands after diapering a child.”
“There were no updated feeding instructions for 4 infants.”
“During the inspection the outside play area had a Little Tykes play scape that had broken plastic pieces, a bike had a hole in its seat exposing the material. *This was corrected at inspection when the broke items were removed.*”
“During the inspection a gate on the play ground was locked using a combination lock. No one at the operation knew the code to unlock it. *This was corrected when the lock was cut off the gate.*”
