Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Most current fire inspection was not available for review.”
“I observed a caregiver that was being counted in ratio, walk out of the classroom, leaving the children in her room unsupervised. This was corrected when the caregiver walked back into the classroom.”
“A staff was present, being counted in ratio and supervising a group of children by herself, without the staff being cleared by the background check unit as eligible.”
“Two staff were unaware of the age group of the children that they were responsible for.”
“Operation failed to complete mandatory report by the due date.”
“Operation did not address their procedures on supporting inclusive activities for children with special care needs.”
“An employee picked up a child inappropriately by the arm, causing an injury to the child.”
“I observed an unlocked cabiinet that stores several cleaning supplies. The children in this room are big enough to reach the cleaning products.”
“A caregiver did not wash her hands after changing two children's diapers.”
“Two children's hands were not washed after having their diaper change.”
“I observed a slide in the outdoor play area that is 31 inches in height, that did not have the required surfacing depth below.”
“A child's Food Allergy Emergency Plan was not signed by child's parent.”
“Upon reviewing children's records, there was no healthcare information.”
“A new employee was present and had direct access to children. Employee was being counted in ratio. Employee does not have any of the required background checks. Employee left the operation.”
“Upon reviewing children's information, parent did not authorize emegency care.”
“I observed two bookcases in two diifferent classrooms that had a bookcase that was wobbly. Children were observed napping near the bookcases.”
