“The food served was not reflective of the posted menu. The menu stated, breaded chicked, ground beef, apples According to staff, children were served chicken sandwiches, corn, and fruit.”

“A slide was observed on the playground with no loose-fill or unitary surfacing.”

“In the infant and toddler rooms, caregivers personal bags, were observed to be within reach of children.”

“The operation did not have a current fire inspection. The last available inspection was dated 10/2024”

“The fire alarm system sounded multiple times on and off during the inspection. There a notice on the alarm box that stated " trouble smoke". Last doucmented inspection from the alarm company was dated 11/15/2024. NOTE: According to director the alarm will sound when it is windy.”

“The playground was not observed to be maintained. Pieces of plastic, styrofoam cups, paper, torn rugs and fabric was on the playgroud. There was an unknow wire on the groud, leading away from the playground. A slide was observed to be cracked, Two basketball goals were lying on the groud, surrounded by trash. Several children were observed to be lying on torn/broken nap cots.”

“Of the 6 staff records reviewed, 5 staff including the director did not have the preemployment affidavit completed and on file for review.”
