Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child in care did not have an immunization record available for review.”
“It was noted that one of the children's records was missing the physician's information. Note: During the inspection, the caregiver obtains this information and enters it on the admission forms.”
“It was noted that the phone number list was not available during the inspection. However, the caregiver provided the phone list during the inspection and posted it next to the phone.”
“It was observed that the date of admission was missing from two children's records. Note: The caregiver added the admission date to the records during the inspection.”
“The operational policy did not include the information of indoor and outdoor physical activity.”
