A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“A follow up was conducted on 7/13/2026. The operation had a gap in the bottom of the door blocked with a blanket.”
“Two chemicals were in reach of children under the sink in the kitchen. This was corrected at inspection when both chemicals were removed.”
“A potty chair and a toilet both had urine in them during the walkthrough. This was corrected at inspection when both toilets were cleaned.”
