Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The training certificate that was provided did not meet the requirements for first aid certification.”
“Caregivers were observed in the classroom closet with their back to the children and engaged in setting up nap mats while children were eating lunch with a child in the classroom with food intolerances.”
“During the investigation, it was found that a staff member did not have a current first aid certification.”
“During the course of the investigation, it was observed that a child wearing an oversized tshirt had the shirt pulled over the back of the chair they were sitting on to keep them secured in the seat.”
“The operation failed to complete an incident report after a child received an injury while in care that required medical attention.”
“Upon review of the operation's emergency preparedness plan it was missing information on how children who were non-mobile will be evacuated, the evacuation location if children and staff can not return to the operation, and how continuity of care will be maintained at the relocation site.”
“Emergency drills have not been documented for 2026.”
“There was a black storage container on the larger playground with a lid that was not secured to prevent accidental closure.”
“An over the counter medication kept at the operation for a child enrolled expired 11/2024.”
“Two medictions stored at the operation did not contain the original prescription for the medication.”

“A television in the dramatic play area was at the children's level and not anchored to avoid possibly being pulled down off the shelf.”
“The operation did not have a current sanitation inspection available for review. The last recorded inspection was conducted on 1/10/23.”
“The slide on the climbing unit is currently taped off at the top and bottom of the slide. The bottom of the slide has two broken areas where children can be injured or clothing could potentially get caught on. There are also numerous tree roots exposed on the larger playground that can cause a tripping hazard.”
