Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“I observed a group of toddlers transitioning to the dining area for lunch without washing their hands.”
“I observed a mop bucket with water in the hallway. The bucket was unattended during the walkthrough and accessible to children in care.”
“The loose-fill material on the playground is not at the appropriate depth of at least 6 inches.”
“2 out of 8 staff files is missing annual training for Emergency Preparedness.”
“1 child with a diagnosed food allergy needs an updated plan signed by both the parent and physician.”
“The support posts for the composite play structure needs to be marked, indicating the depth of loose fill.”
“The playground has 1 active ant bed that needs to be treated.”
“While at the operation it was observed that the loose fill on the playground was less than 6 inches deep around the slide and play structure.”
“It was found that the number of employees who ceased working at the operation in 2023 was not reported through the provider portal by 1/15/24.”
“Upon arrival, the director had not arrived yet, and no qualified caregiver was left in charge of the operation for approximately 10 minutes. This was corrected when the designated caregiver returned to the operation.”
“During the follow up inspection it was found that the toddler room does not have proper furnishing for children to sit and do activities. The caregiver placed children in highchairs and provided them a book to read.”
“It was found that the dosage administrated was not documented in the medication log.”
“The medication bottle does not show the date of when the medication was brought to the operation”
“Six children's records were reviewed. Three of the records do not have current immunization records based on the child's current age. NOTE: This deficiency was corrected at inspection.”
