Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children's records lacked the health statement. One child's record lacked it from the parent, and the other laced the health statement from a health care professional.”
“Two children's records lacked the updated immunizations for the children's age.”
“The personnel training records were inaccessible during the inspection as some of the training records were stored electronically.”
“The admissions form requested was signed and filled out after the date of the inspection on 6/27/25.”

“Based on the information obtained during an investigation conducted by CCI, there were 6 infants, with a specified age group of 12- 17 months, with one caregiver present in the infant room.”
