Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the information obtained during the DFPS investigation, the operation did not share the incident report with the parent, and the parent did not sign the incident report within 48 hours as required.”
“Based on the evidence obtained during the investigation there is sufficient information to prove that a child was unsupervised for approximately 10 minutes when he escaped from the gate on the playground.”
“Based on the information obtained during the investigation there is sufficient information to show that the caregiver was over ration by one child prior to the incident occurring.”
“In the toddler room the outlet plate is loose from the wall. The diaper changing mat is ripped causing it to not be sanitizable. Note: Pictures were taken of the mat and a video was taken of the outlet plate. The mat was ordered before I left the inspection. The outlet was tightened during the inspection.”
“Based on the preponderance of information obtained during the investigation there is sufficient information to prove that a caregiver did not use good judgment by not confirming the child had removed their hand from the door before allowing the door to close on the child's hand.”
“Based on the preponderance of information obtained during the investigation there is sufficient information to prove that the caregiver did not use good judgment when placing duct tape around a child's shoes in an attempted to keep the shoes on the child's feet. Note: This was corrected prior to my inspection at the operation when the director had a meeting with the caregiver, given a written disciplinary, suspended the caregiver, and given the caregiver new responsibilities in a different classroom.”
“Three children files out of six enrolled did not have completed immunization records. Note: The director was able to print one child's updated immunization record off of imtrac.”
