Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A sampling of children records were evaluated. Three children records lacked emergency care transportation. Two children's records lacked documetation of field trip permission. Two children records lacked emergency care authorization. Provided a list of the children and their missing information to the directors at inspection.”
“The operational policies were evaluated. The policies lacked the Parents Rights.”
“Safety documentation for emergency drills, fire extinguishers, and smoke detectors was not available for review at inspection. These are kept on file electronically and the computer was down.”
“This standard was reviewed as part of an investigation and found deficient. The director did not ensure that the operation was in compliance with MSR when she placed an employee in a classroom without confirming documentation of qualifications, and when she failed to ensure that all caregivers were complying with discipline standards.”
“This standard was reviewed as part of an investigation and found deficient. One caregivers personnel record lacked proof the caregiver meets the education qualification.”
“One caregiver did not have documentation of pre-service training on file.”
“This standard was reviewed as part of an investigation and found deficient. It was determined a caregiver has handled children in a rough manner and that a second caregiver grabbed a child roughly and placed her facing the wall. Both caregivers are no longer employed by the operation--one having been terminated and the other resigned.”
