Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of the seven files reviewed did not have a notarized affidavit available for review.”
“Documentation of drills for September 2025 were not available during inspection.”
“A diaper changing table had a torn changing mat. Note: This was corrected at inspection when caregiver changed the diaper changing mat.”
“Operation Mustard Seed Academy failed to notify Child Care Regulation within the required two-day timeframe after receiving non-compliant lead test results.”
“The facility's fire inspection has expired; it was last updated on 9/12/2023.”
“The director did not have current training in Pediatric First Aid.”
“The Child Product Safety Commission form was not available for review. This was corrected at inspection when the owner signed the form and posted it.”
“The annual fire inspection was last conducted on 01/26/21 which is past the required twelve months.”
“In the Toddler room, there were three bottles in the refrigerator that were not labeled with the children's names. This was corrected at inspection when the labels with the children's names were added to the bottles.”
“A television was observed in the 4-5 year old classroom and was not anchored to the wall or furniture. NOTE: This was corrected at inspection by television being removed from the classroom.”
“A truck on springs was observed on one of the playgrounds without the proper 6 inches of loose fill in the fall zone.”
“Six out of eight children's records were lacking an emergency contact address.”
