Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The Children's Product Certification had not been verified in over a year.”
“The center did not have criteria for extreme weather in operational policy.”
“The operation did not have a policy that included children with special care need in their operational policy.”
“2 out of 8 children did not have a doctors signature after being enrolled for over 12 months at the center.”
“One out of the five childrens records evaluated did not have an emergency contact address. Note: This was corrected at inspection.”
“A classroom had 10 children in a classroom with one caregiver with the specified age group of 12-17 months. The classroom was out of ratio by 5.”

“The operational policy was lacking the outdoor policy.”
“The operational policy was lacking the safe sleep policy. Corrected at inspection: The infant safe sleep policy was provided during the inspection.”
“Two out of ten children's records reviewed were lacking the doctor signed health statement within the one year of enrollment.”
“Four out of the ten children's records reviewed needed an updated immunization record.”
“The center added 4 rocking cars into the playground and they were only 6 feet apart and not the required 12 feet a part from each other.”
“Four classrooms were observed to be out of ratio. One infant classroom had a specified age group of 12-17 months and 10 children in care. The classroon was out of ratio by five children. The second classroom had a specified age group of 0-11 months and had 6 children in care. They were out of ratio by one child. The third and fourth classroom had a specified age group of 2 years old and had 14 children, they were out of ratio by 3 children in each classroom.”
“The center was not allowing parents to go past the entrance into the classrooms.”
