Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Primary Caregiver, who was counted in ratio, left three children in care asleep on their mats and stepped outside of the operation, leaving these children unsupervised.”
“During a DFPS investigation, it was determined a sleeping child was left unsupervised in the transportation vehicle for approximately 15 minutes. The caregiver was aware the child was asleep, decided to allow the child to remain in the vehicle and went into the operation.”
“During a DFPS investigation, it was determined that two enrolled children did not have admission information.”
“During the follow-up inspection the operation was observed out of ration again. Primary caregiver had 11 children in care. There was an infant 0 to 17 months old, (2) two years old, (7) 18 months to three years old, and (1) four-year-old. The operation was over by two children in the age range 18 months to three years old. Corrected during the follow-up inspection when a 17-month-old and (2) three-year-old children were picked up leaving the operation in compliance with child/caregiver ratios.”
“Two play-yards used for infant sleep were observed with two additional pads besides the original pad that came with the play-yard. Corrected during this inspection, additional pads were removed from the play-yards.”
“During the inspection a nine-month-old infant was observed in a baby walker. Infant was removed from baby walker and baby walker was removed from care area.”
“Operation had (two) children 0-17, (seven) children 18 months through three years old, and (three) 4 years old and older. Corrected during the inspection when four children were picked up by their parents, leaving the operation in ratio.”
“Fire safety form indicated operation conducted only two sheltering/severe weather drills in 2022. A severe weather drill was conducted and documented during this inspection.”
“Fire safety form indicated operation conducted only two lock-down drills in 2022. A lock-down drill was conducted and documented during this inspection.”
“A five-month-old infant was missing monthly feeding instructions since May 2022.”
“A five-month-old infant was observed sleeping in a play-yard on top of a pillow-like positioner covered with a blanket. Primary caregiver removed all items immediately out of the play-yard during the inspection.”
“Fire Safety Practice form indicated last fire drill was conducted February 2022. A fire drill in less than three-minutes was conducted and document during this inspection.”
“Six-month-old infant was missing monthly feeding instructions for February 2022. Infant file had monthly feeding instructions dated August 2021.”
“Upon arrival at the home operation at 10:30am a parent picked up a six-month-old infant from her crib and placed her in a car seat in preparation to depart. Infant was in the crib covered with a blanket, on top of another blanket. Corrected during the inspection when parent took infant out of her crib with her blanket. Caregiver removed the second blanket during this inspection.”
