A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“3 children did not have a physician statement on file.”
“One caregiver did not have an active CPR/First aid card.”
“Information obtained throughout the investigation shows that the operation has an ongoing pest control issue. Mouse droppings were present on the floors and furniture in areas used by the children. The operation is working with a pest control company to resolve the issue.”
