“A caregiver was observed diapering a child utilizing gloves. Once completed the gloves were removed and the caregivers hands were not washed before applying new pair of gloves and completed another child's change. Noted: this was correct immediately when the caregiver was instructed to wash their hands.”

“An infant was observed seated in a highchair and the safety straps were not being utilized. Noted: This was corrected immediately when the safety straps were fastened.”

“A caregiver gave incorrect number of children she was responsible for during naptime. Caregiver had eleven sleeping children in her class, yet stated she had ten.”
