Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Observed the playground without the appropriate amount of loose fill.”
“Observed expired emergency medication stored in the medication cabinet. NOTE: This corrected by being removed and returned to the parent.”
“Observed a changing mat cover with a tear making the mat absorbent.”
“The operation had one caregiver present who was left in charge due to the director's absence. This caregiver did not have an eligible background check.”
“A caregiver was caught on video improperly feeding an infant in an unsafe manner.”
“A caregiver altered beyond the manufacture instructions for an infant's bottle. The caregiver cut the nipple of the bottle off and added solid foods to the bottle. The bottle was forcefully fed to a 6-month-old.”
“One out of six children files reviewed did not have a health statement available to review.”
“Based on the results and preponderance of evidence of the DFPS investigation, there is sufficient evidence to support prohibited punishment with a caregiver grabbing a child in care.”
“Based on the results and preponderance of evidence of the DFPS investigation, there was sufficient evidence to support a child was physically abused by a caregiver.”
“Based on the results and preponderance of evidence of the DFP investigation, there was sufficient evidence to support corporal punishment by a caregiver.”
“There was not a current annual fire inspection available to review.”
“There was not a current annual sanitation inspection available for review.”
“One out of six children files reviewed did not have a health statement available for licensing to review.”
“Four out of six children files reviewed needed an updated immunization record.”
“A caregiver who did not have an eligible background check was present at the operation and had access to the children in the classroom. Note: This was corrected at inspection when the director had the caregiver to leave.”
