Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the investigation, it was found that staff did not wear gloves when handling blood from a child who was injured.”
“The fire extinguishers were last serviced February 2025.”
“A caregiver caring for children under the age of 24 months lacked documentation of current SIDS/Shaken Baby Training.”
“Two out of ten children's files evaluated lacked an updated immunization record on file.”
“Based on information gathered, a caregiver failed to use good judgment when she is seen on video forcefully sliding a child's chair, resulting in the child falling and hitting the table. In addition, the caregiver and another caregiver failed to use good judgment by using a pointer "stick" to hit the table to scare the children.”
“On a playground, the metal covering around the drinking fountain was detached from the wall, exposing sharp metal. Note: This was corrected when the metal was attached back to the wall. Some drains on the playground were lacking appropriate sealant/ caulking, which resulted in gaps being exposed.”
“The operation did not follow the background check provisional conditions of a caregiver. Note: This was corrected when a second caregiver was added to the class.”
“Personnel files were not complete/available within a reasonable amount of time. Inspectors waited over three hours and all files were not completely provided.”
“A two month old, who is unable to roll over was observed sleeping on his stomach. Note: The caregiver rolled the child to his back when it was brought to her attention.”
“A two month old was observed sleeping with a burp rag in the crib. Note: The burp rag was removed when it was brought to the caregivers attention.”
“One caregiver was briefly observed with eleven children at nap time with a specified age group 12m-17m. Note: This was corrected when a second caregiver entered the room.”
“The consumer product safety recall was last certified January 3, 2022.”
“A caregiver was observed grabbing several children by the arm.”
“Diaper cream and sunscreen were observed in the bathroom and were accessible to the children. Note: This was corrected when the caregiver removed the items.”
“One out of ten children's files evaluated lacked an updated shot record on file.”
“Two caregivers lacked an employee affidavit on file.”
“One out of ten personnel files evaluated lacked current SIDS/Shaken Baby training.”
“Two out of ten personnel files evaluated lacked current first aid/CPR training.”
“Caregivers did not use positive statements when redirecting children from climbing on the tables.”
“Two out of ten children's files evaluated lacked a special needs statement.”
“Based on information gathered through a DFPS investigation, a caregiver did not know the names of the children she was responsible for. A child was crying when two other children were on top of him. The caregiver did not know the names of the children involved and did not check the child for any injuries.”
“One caregiver did not wash their hands after changing a child's diaper.”
“One child did not wash their hands after having their diaper changed.”
“The caterpillar play structure on the playground was observed to be cracked. Note: This was corrected when a caregiver removed the caterpillar from the playground.”
“Two out of ten children's records evaluated lacked an updated shot record on file.”
“The infant classroom was not utilizing their individual storage when all backpacks/ diaper bags were stored on the floor, accessible to the children. Note: This was corrected when the caregiver removed the backpacks/ diaper bags from the floor and placed them on hooks.”
“Based on information gathered during a DFPS investigation, a caregiver was observed and admitted to draping her leg over children who were laying on their cots to force them to remain still and stop getting up during naptime in order to control their behavior.”
“Based on information gathered during a DFPS investigation, a caregiver was observed by other caregivers and admitted to yelling at children.”
