Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A wall in the school-age room had a hole that required repair. Two toilets had not been flushed or cleaned from the previous business day. During inspection, the toilets were flushed and cleaned by a caregiver.”
“Two caregiver files were missing affidavit 2912.”
“One caregiver did not obtain CPR training within 90 days of employment.”
“A caregiver was using a personal cell phone to view social media while supervising children during naptime.”
“A caregiver attempted to force a child to keep their head on their mat during naptime.”
“A caregiver lightly hit a child on the head to instruct them to keep their head down on their mat during naptime.”
“Active play equipment within a circular playground with loose-fill did not have the required 6 feet of use zone in all directions.”
“The school-age room did not have the emergency evacuation and relocation diagram posted. The caregiver posted the diagram prior to end of inspection.”
“Three infants not yet eating table food did not have updated written feeding instructions.”
“During a DFPS investigation, the investigator observed children out of control in classrooms. It had been observed that children were jumping from chairs, and caregivers were not intervening to correct the behaviors.”
“The operation did not report a known allegation of inappropriate contact between two children.”
“Various medications were present that were not assigned with a child's name and did not have the date the medication was brought to the operation. The medication was removed from the operation during the inspection.”
